St Oswald’s Hospice provides specialist care and support for babies, children and young adults aged 0-25 years.

Our Children and Young Adults Service offers Residential Short Breaks, Outreach Services and End of Life Care for young people with progressive, life-limiting conditions.

We also support the whole family including parents, siblings and carers.

We care for young people from Northumberland, Newcastle, Gateshead, North Tyneside, South Tyneside, Sunderland, Durham City, Chester-Le-Street and Derwentside.

What is the referral process?

All referrals must be made by a health and social care professional (Doctor, Social Worker, Community Nurse or Pathway Coordinator).

To make a referral for our Residential Short Stays or Outreach Service, please fill out and submit a referral form by clicking the buttons below. You will also need to supply the following to childya@stoswaldsuk.org

• Health Assessment
• Social Assessment

An assessment and referral pathway has been agreed with health and social care colleagues in each area. We also worked closely with colleagues at the Great North Children’s Hospital and Newcastle upon Tyne NHS Trust.
