Outreach Service

We provide nurse-led Outreach Services to children and young people in their homes and local community.

This specialist service extends our care to young people away from the hospice, as part of an agreed care package.

Our Outreach support is tailored to each family and circumstance and can be provided on a regular, long-term basis or in acute situations.

Outreach care packages are reviewed regularly with wider health and social care teams.

Extending our care to home and the community

  • Our trained nurses and support workers deliver specialist and individualised care to a child or young adult and their family in the comfort of their own home.
  • Outreach care can also take place outside the home, including support to access fun and appropriate activities in the local community.

Frequently asked questions (FAQs)

Yes. It is possible to access our Outreach Service even if your child or young adult doesn’t already use our services.

We offer Outreach Services to children and young adults aged 0–25 years.

We can provide Outreach Services to families from Northumberland, Newcastle, Gateshead, North Tyneside, South Tyneside, Sunderland, Durham City, Chester-Le-Street and Derwentside.

This will depend on service capacity and the care package agreed on by the wider health and social care team. All care packages are reviewed regularly.

Trained St Oswald’s Hospice nurses and/or support workers depending on your child or young adult’s needs. This will be determined as part of the agreed care package.

We aim to provide a responsive service and can, at times, offer Outreach on a short-term basis in an acute situation.

This is dependent on service capacity and availability.

A child or young adult must be referred to our Outreach Service by a GP, Social Worker, Community Nurse, or Pathway Coordinator.

To make a referral, health and social care professionals should fill out and submit the referral form below. They will also need to supply the following to childya@stoswaldsuk.org

  • Health Assessment
  • Social Assessment

An assessment and referral pathway has been agreed with health and social care colleagues in each area. We also worked closely with colleagues at the Great North Children’s Hospital and Newcastle upon Tyne NHS Trust.
