Sometimes it’s hard to talk to those closest to you about illness, death and dying. However, you want them to be prepared should you become too unwell to communicate your wishes, or when you die.
‘What I Want You to Know’ was created with you and your trusted people in mind – because people have told us they find it really helpful to have everything in one place.
This digital print at home version is a way of recording important information and sharing anything your trusted people might need to know. Think of it as an act of love and kindness to help them cope at a difficult time.

Once you’ve completed your purchase, we’ll send you a link to download a digital copy of our What I Want You to Know booklet. This is a 87 page printable file designed to be printed and written in by hand at home.
Please note that this is not a fillable PDF. To use the booklet as intended, you will need to print the pages before writing in your responses.
For the best results, we recommend printing on a home printer using standard A4 paper.
Be sure to save your file after downloading if you won’t be printing immediately.
What’s Inside ‘What I Want You to Know’…
This 87 page, full colour print at home booklet is a practical guide for you to print and then fill in with your information. It has space to include your wishes for how you want to be cared for in the future and what you would like to happen after you die. It allows you to record the details of your life in one place. It also provides information to help your trusted people to make difficult decisions on your behalf and explains some of the practicalities they will have to consider.
It is divided into clear sections making it easy to use. There are prompts to guide you and space to write by hand. You can complete as much or as little as you like, in your own time. It’s helpful for anyone, whether you’re well now or living with illness.
You’ll find sections about:
- Your personal details and key contacts
- Your care plans/wishes
- Your ‘life admin’ (your Will, Lasting Power of Attorney, bank details, utility providers like gas, electricity and water, subscriptions such as TV, mobile and magazines etc)
- The things that matter most to you (from your faith/beliefs to the pets and people who rely on you and even what to do with your ‘digital legacy’ like your social media pages)
- What needs to be done when you die (who to tell, registering your death etc) and any plans/wishes for honouring or celebrating your life in the way you want.
