Referrals for children & young adult service
We provide specialist residential short breaks and palliative care for children and young people who have progressive health conditions that are likely to result in shortened life expectancy, or are currently experiencing a life threatening episode.
Who can refer?
Referrals for residential short breaks are not made directly to us. There is an assessment and referral pathway that has been agreed with healthcare, social care and St Oswald’s in each area.
To refer a child or young person for a health and social care assessment, please download our ‘Information for Health Care Professionals’ leaflet for more details and then contact your relevant Pathway Coordinator.
If you are referring a child or young person for palliative or end of life care, please contact St Oswald’s directly on 0191 246 9060 and ask to speak to a member of the care team to discuss the case. You must also contact the Pathway Coordinator to discuss the referral.
