Annual assessment clinic for young adults

A new service aiming to support young adults with palliative care needs throughout and following transition from children's services

In addition to our short break service, we are piloting an annual outpatient assessment clinic to support young people aged 16-25 years living with a life-limiting condition.

The new service aims to support young adults living in Newcastle and Gateshead with palliative care needs throughout and following transition from children’s services. The clinic will provide an opportunity for symptom management, signposting, goal-setting and advance care planning.

As this is a new service the first phase of project will focus on patients with neurodegenerative conditions or other neuro-disability. We will aim to widen the service to young adults with other conditions as the service progresses.

The service is in partnership with NHS Newcastle Gateshead Clinical Commissioning Group and funded by Together for Short Lives’ Improving Transitions for Young People Fund.
