Improving transitions for young people with life-limiting conditions and palliative care needsSt Oswald’s Hospice, in partnership with NHS NewcastleGateshead CCG, would like to invite you to attend a breakfast meeting to launch a new project to improve transitions for young people aged 16-25 living with a life-limiting condition.

The aim of the morning is to bring together healthcare professionals from across the region to hear more about the project and how to refer in and get involved.

During the event, you will hear from influential speakers about the project, funded by Together for Short Lives’ Improving Transitions for Young People Fund, plus information on further opportunities for closer working and education.

Keynote speaker

Professor Sir Alan Craft

Professor Craft, previously the head of Newcastle University’s Department of Child Health and now Emeritus Professor, led the research team which initially proved the need for children’s hospice services within the North of England.  Professor Craft’s presentation will focus on transitions to adulthood for young people living with life-limiting conditions and palliative care needs.

Emeritus Professor of Community Child Health, Newcastle University.

Dr Jo Elverson
Consultant in Palliative Medicine, St Oswald's Hospice.

Dr David Jones
Clinical Director for Young People and Families, NHS Newcastle Gateshead Clinical Commissioning Group. 

The event will be hosted by James Ellam, Chief Executive, St Oswald's Hospice.
